Henry Collins - 49th NC, Co I, ANV - Civil War

I sent off a form NATF 80 to the National Archives.  Believe it or not they do have confederate records.  For a small fee of $10 and six weeks later I received the following.  His medical record when he died and also his various receipts for clothes, muster rolls etc.

His medical record - The translation of his disease is basically a gunshot wound through the abdomen - gut shot
